| OTHER NECESSARY VACCINATIONS Lyme disease vaccination is available for dogs and highly recommended in our area. We also offer Bordetella (Kennel Cough) for dogs in boarding and grooming situations. Outside cats should receive Feline Leukemia vaccination. Any of these vaccinations can be added to the above plans at the indicated discount rate. |
| Cats not receiving Feline Leukemia vaccination will only need their distemper combination and rabies vaccinations every 3 years once they have had the proper booster series. Therefore some years the wellness plans will not need to include vaccinations for cats. There will be a dual cost of plans for cats. |

There is no initiation fee for any of the packages.
Plans can be paid in full via check, cash, debit and credit cards.
Extended payment options (6 or 12 month repayment plans-some interest free) are available
through Care Credit if you qualify.
We are unable to offer refunds should you elect not to take advantage of all products and
services. All products assume availability and if necessary we will substitute appropriate
products. Wellness plans assume your pet remains on schedule with its vaccinations. If not,
additional boosters may be needed and are the responsibility of the pet owner.
